sw: acuthin.exe.....application error??

We are converting a working process that displays images using Microsoft Reporting Services images inside of an imbedded browser component to display the same images using PDF format. The process is working correctly until the user closes the application....once they close the application they receive the following error (sorry for typing as I could not figure out how to add a screen print to the post!):

sw: acuthin.exe - Application Error
The instruction at "0x0477609c" referenced memory at "0x0000000014". The memory could not be "read".
Click on OK to terminate the program
*********************

Has anyone seen this issue before and did you find a solution?

    Is your application running inside the browser when you close the browser? Is that when the mav occurs?
    Or are you running your application via thin client and when you close the window that is presenting and making a pdf, that the mav occurs?

    Application is running in Thin-Client when the user selects to EXIT application. After the screens are removed the error appears.

    I suggest contacting Customer Care, the COBOL application should not mav on exit.
