Problem:

The user selects 'Prepare' from the menu bar, but the CRUD Report is not available in the list of available options.

Resolution:

The CRUD Report may not be available for a couple reasons.  Please check the following:

1.  Verify that CRUD Report is checked under Tools > Customize Menu.
2.  User must have Admin Rights.
