How does one install ccitcp2 when using Server Express 4 onwards?

Problem:

ccitcp2 is not installed when one opts to install Enterprise Server during the Server Express Install procedure.

Resolution:

If one installs Application Server or a Server Express development system without accepting the Enterprise Server (ES) option, ccitcp2 is installed.

If one responds 'yes' to installing ES or installs a stand-alone ES, ccitcp2 is replaced with a script that runs mfds.
