    Yes, Patch Updates are cumulative so Patch Update 2 will contain all fixes from Patch Update 1, etc.

    ...also if we update visual studio v3 to patch 3 do we have to update COBOL SERVER on all client machines before deploying our new exe?

    Yes, the COBOL Server product should be at the same or higher version level than the development version that the programs were compiler with.
